Co-Pays

• Mental Health Copay – Effective 09-01-11 there will be a $25 copay for all mental health office visits.

• Chiropractic Copay – Effective 09-01-11 the chiropractic copay will increase from $13 to $25.

• Emergency Room Copay –Effective 09-01-11 the emergency room copay will increase to $100 for all visits to contracted providers. If you are admitted to the hospital, the copay will be waived.

• Medical + Physical Therapy Copay Cap – Currently there is not a cap on the number of copays paid by the patient.  Effective 09-01-11 a copay maximum of 50 copays per calendar year will apply.  Once the copay maximum is reached you will not be responsible for additional copays in that calendar year.

Prescription Co-Pays
As a reminder, in accordance with the Health Plan changes effective 09/01/07, prescription Co-Pays will increase by $1 for a 30 day supply and $2 for a 100 day supply.  Effective September 1, 2011 pharmacy Co-Pays for the 2011/12 year will be as follows:
