Prescription Benefits

As part of the CUSD Health Plan, prescription coverage is provided through Caremark, Inc. with many pharmacies to choose from in the Fresno area as well as across the United States. There is a $5.00 co-pay for generic prescriptions, $20.00 co-pay for preferred brand drugs (Performance Drug List), and a $30.00 co-pay for brand name prescriptions.

Employees and their dependents who routinely take maintenance drugs have another option for filling these prescriptions during their first two fills that will save them time and money. Caremark provides the District with a mail order service that employees can utilize to obtain 100 day supply of medication for the price of a 60 day supply ($10.00 generic, $40.00 Preferred Brand Name, and $60.00 brand name). After a member receives two fills of one maintenance medication it is mandatory that they use the mail order program for all subsequent refills.

You can print order forms from Caremark.com. Simply log in to Caremark.com with your username/login ID and password, click on the "Prescriptions & Coverage" tab, then select Print Forms from the menu on the left side of your screen. You can also contact Customer Care at customerservice@caremark.com, contact them via your toll-free number or contact your benefits office.
