Submit Your Training FormDone electronically through InformedK12. Use a reliable email (and make sure it's spelled correctly!) when initiating the form, as you will receive your training instructions via an InformedK12 email. FOR THOSE PREVIOUSLY TRAINED: Please make sure to have a copy of your certification card available to upload. If you do not have a copy of the previous certification you received through CUSD, please email carlyregehr@cusd.com for assistance.

Training AssignmentWhen you submit your form, it will route to Risk Management, who will assign your training by sending your form back to you via InformedK12. Look out for an email from InformedK12 that says "New Certification OR Recertification Instructions Available on Page 2." Read the message on the email, then click the "VIEW YOUR FORM" button in the email to open your training form & review your instructions. Coaches will have their additional instructions provided on page 2.

Complete Your TrainingNew Certifications: Hold onto your form until your in-person course is finished, then send your form to your instructor. Please refer to page 2 of your form for more details. Recertifications: You will receive an additional link from HSI via email with part one of your two-part training. After completing your HSI course, please refer to page 2 for instructions on getting skills tested. Coaches: Complete your additional training requirements PRIOR to submitting your form to your instructor.

Skills TestingTo submit your form to your instructor, click the red "I have reviewed this form" button either at the top (mobile) or bottom (desktop) of your screen & choose the "Send to Next Approver" option. Find your instructor's name & submit. New Certifications: Your skills testing will be conducted during your in-person course. Recertifications: Make an appointment with a certified instructor & send your form AFTER they have confirmed an appointment with you.

Certification CardUpon completion of your skills testing, your instructor will send the form directly to Risk Management on your behalf. Keep a lookout for an email from InformedK12 titled "Certification Card for Adult CPR, AED, and First Aid." Click the access link provided to route back to your form & look on page 5 for your certification card. It is YOUR responsibility to provide your certification card & additional coaching certificates of completion (if applicable) to your site.
