Adult First Aid CPR/AED

Step 1:
Submit Your Training Worksheet
To start the Adult First Aid CPR/AED training process through the District, please submit a Training Worksheet through Informed K12. You can find the link to our Training Worksheet by returning to our CPR Home Page. Make sure to utilize a reliable email (and the email is spelled correctly!) when initiating your Worksheet, as you will receive all your training instructions via an email from Informed K12.
If you are recertifying a previous certification card that is less than 6 months expired, please make sure to have a copy of your certification card available to upload to your training worksheet to show proof of eligibility for recertification.
If you were trained through the District previously and need a copy of your most recent certification card, please email CarlyRegehr@cusd.com for assistance.
When you submit your Worksheet, it will automatically route to the Risk Management office, where a staff member will review your Worksheet and assign your training to you based on the information provided. Once your training is assigned, you will receive an email from Informed K12 directly with a subject line that starts with “ACTION REQUIRED,” followed by your training assignment.
Step 2:
Complete Your Training
Please carefully review all the instructions provided in the email from Informed K12 that starts with “ACTION REQUIRED.” Make sure to save that email because you will need to use the “VIEW YOUR FORM” button at the bottom of the email to upload any required documents (if applicable) and to send your Worksheet to the instructor who will complete your skills testing.
If you have any questions on what to do next, your training instructions should provide information on who to contact.
Step 3:
Skills Test with a Certified Instructor
NEW CERTIFICATION TRAINEES: You will complete your skills testing with the instructor teaching your in-person course.
RECERTIFICATION TRAINEES: You will need to contact a certified instructor through CUSD in order to complete skills testing. Please refer to your training instructions in your Informed K12 email for step-by-step information on how to proceed.
After you complete skills testing with your instructor and send them your Worksheet, your instructor will sign off on the form and automatically it submit it to the Risk Management Office on your behalf.
Step 4:
Download and Save Your Certification Card
Upon receiving your Training Worksheet from your skills instructor, the Risk Management Office staff will issue your certification card. Your certification card will be uploaded directly to your Worksheet. You will receive an email from Informed K12 titled “Adult First Aid CPR/AED Certification Card Attached” with a link to access your completed Training Worksheet.
Your certification card will be available at the bottom of Page 2 of your training worksheet, where you can download it and save it as a PDF. This card is valid for two years, and the expiration date will be listed in the same section where you can download it. You may renew your certification with the District as long as it has not been expired for more than 6 months.
Once you receive your certification card, you must provide it to your Office Manager if you work at an elementary school OR your Athletic Director if you work at an intermediate or high school. If you are unsure how they would like to receive it, please contact them for further direction.
