Employees now have the option of submitting Payroll forms electronically through Informed K12.
- When submitting forms through IK12, please only use your @CUSD email address.
- The Direct Deposit/Rapid PayCard Authorization Form is excluded from electronic submission and therefore not available on IK12
Additionally, all Payroll forms can be downloaded below and submitted in person to The Payroll Department. Forms are also available in the Payroll office for employees to fill out and submit. To make an appointment or for questions regarding Payroll forms please contact Payroll at 559-327-9074.
Payroll cannot accept e-mailed forms, copies of forms, or forms without an original signature. No exceptions.
